CERVICAL SCREENINGS & BREAST SCREENINGS COVERED:
  • Physician office visit for assessment, clinical breast exam and education for women 40 and older.
  • Screening mammogram (includes the radiologist's fee), for women 40 and older unless otherwise specified.
  • Physician office visit for pelvic exam, education and Pap test (includes the lab fee for the smear).  We cannot cover this test if you have had a hysterectomy.
  • Any additional lab tests or procedures done during your pap smear are not covered. This includes, but is not limited to lab tests for Chlamydia, Gonorrhea, HIV and some HPV testing. Please ask your provider what they will be screening for on your Pap test!  You will be billed for these additional lab services if they are done.

COLORECTAL SCREENINGS COVERED:
  • Fecal Immunochemical Test (FIT) done in the privacy of your home. This screening will only be covered if sent to Quest Laboratory in the special envelope that is provided.

BREAST DIAGNOSTICS (Call for approval and confirmation prior to having services performed):
  • All clients that need additional breast follow-up services (i.e. breast ultrasound, breast biopsy, diagnostic mammogram (additional views), etc.) must have prior approval from the Cancer Services Program Staff Only!

CERVICAL DIAGNOSTICS (Call for approval and confirmation prior to having services performed):
  • All clients that need additional cervical follow-up services (i.e. Colposcopy, etc.) must have prior approval from the Cancer Services Program Staff Only!

COLORECTAL DIAGNOSTICS (Call for approval and confirmation prior to having services performed):
  • All clients that need additional colorectal follow-up services (i.e. colonoscopy, sigmoidoscopy, barium enema etc.) must have prior approval from the Cancer Services Program Staff Only!

Please note that appointments to obtain results of diagnostic testing may not be covered by the Cancer Services Program and will be the responsibility of the client.

PATIENT NAVIGATION SERVICES
The Patient Navigator Program assists clients who have had abnormal; clinical findings related to a breast, cervical and colorectal cancer screening. This is accomplished with the assistance of a Patient Navigator who is responsible for identifying barriers to follow up care and assisting with resources and support.

Any client in Eastern Suffolk County who is having difficulty trying to complete follow up care after an abnormal finding is eligible to receive this assistance.

MEDICAID CANCER TREATMENT PROGRAM (MCTP)
Provides optional Medicaid eligibility for low-income men and women diagnosed with breast, cervical, colorectal or prostate cancer or pre cancerous breast and cervical conditions.

Our staff will assist you with this Medicaid application process if you are:
  • At least 18 yrs of age or older
  • Income at or below 250% Federal Poverty Level
  • Not covered under any creditable insurance
  • In need of treatment for colorectal, prostate,  breast or  cervical cancer or pre cancerous breast and cervical conditions
  • Resident of New York State
  • US citizen or alien with satisfactory immigration status
